Understanding health insurance

What it is
Health insurance spreads the cost of medical care across many people. You pay a regular amount called a premium, and in return the plan pays some or all of your covered medical costs, protecting you from bills that could otherwise be financially devastating.
Systems vary by country: some are funded through taxes and cover everyone, some through employer or private plans, and many are a mix.
The key terms
The premium is what you pay to stay covered. The deductible is what you pay out of pocket each year before the plan starts sharing costs. A copayment is a fixed amount per visit or prescription, and coinsurance is a percentage you pay of each bill.
The out-of-pocket maximum is the yearly cap on what you can be charged for covered care - after that, the plan pays in full. Knowing these four numbers tells you what a plan really costs.
Choosing a plan
The cheapest premium is not always the cheapest plan. Compare what you would pay in a typical year and in a bad year: premiums plus likely out-of-pocket costs. Check whether your doctors and hospitals are in the plan's network, and whether your medicines are on its covered list.
Plans with lower premiums usually have higher deductibles, which suits people who rarely use care; frequent users often pay less overall with a higher-premium, lower-deductible plan.
Using it well
Staying in network keeps costs down. Many plans cover preventive care - checkups, vaccinations, screening - at no extra charge, so using those benefits is free health protection. Keep your insurance card and policy documents accessible, and understand what requires pre-approval before treatment.
If a claim is denied, you can appeal, and many denials are overturned. Hospital billing offices and patient advocates can help with large bills and payment plans.
